New Members Registration Procedures

Make sure you have a Sports Engine Account before you start to a purchase USAV membership.
 
Step #1 Create a Sports Engine Account, Click Here
(Sports Engine Account will be in parents name/info)

 

Step #2 Player's USAV Membership Registration, Click Here

Step #3 - Complete the Evaluation Sign Up $50  and at the end pay the $50 Eval Fee.  *Must be a USAV member before you sign up.  You will need your USAV membership number to sign up for upcoming Evaluations.

Step #4 -  Complete the Medical Release Form - 2022/2023 Notarized Medical Release Form (A notary will be present at evaluation check in; you must have your photo ID and the parent must sign the form in front of the notary).
