Required to participate in the team evaluations:

MANDATORY!

1. You must register as a member for USAV, below are the type of memberships needed for the program options available at PIVC.

• 2022 USA Full Membership $90 -Full membership is required for Subregional, Regional, Power, & Elite teams. (Print card after you have registered online and bring a copy with you to evaluations)
• 2022 USA Developmental Membership $50 - Is used for Developmental Program/Grassroots program (Print card after you have registered online and bring a copy with you to evaluations)
• 2022 USA Developmental Membership $Free  Ages 8 & Under Click Here - Is used for Developmental Program/Grassroots (Print card after you have registered online and bring a copy with you to evaluations)

2. Complete the Evaluation Sign Up  and at the end pay the $40 Eval Fee.  Must be a USAV member before you sign up.

3. 2022 Notarized Medical Release Form (A notary will be present at evaluation check in; you must have your photo ID and the parent must sign the form in front of the notary).
