Evaluation Registration Procedures

Make sure you have a Sports Engine Account before you start to a purchase USAV membership.
Step #1 Player's USAV Membership Registration, Click Here
You will need a SportsEngine Account to complete the USA Membership.
 
Create a Sports Engine Account, Click Here
(Sports Engine Account will be in parents name/info)
 

Step #2 - Complete the Evaluation Sign Up $60  and at the end pay the $60 Eval Fee.  *Must be a USAV member before you sign up.  You will need your USAV membership number to sign up for upcoming Evaluations.

 

Step #3 -  Complete the Medical Release Form - 2023/2024 Complete the Medical Release Form - 2023/2024 (A notary will be present at evaluation check in; you must have your photo ID and the parent must sign the form in front of the notary).

Types of Memberships

11u - 18u Must choose 23-24 Gulf Coast Junior Player Membership
 
Grassroots players can choose from one of the 3 memberships.
23-24 Gulf Coast Developmental 12U Player (12u Grassroots)
 
23-24 Gulf Coast Junior 8 & Under Player (8u Grassroots)
 
23-24 Gulf Coast Junior 10 & Under Player (10u Grassroots)
